SANTA CLARA -- Quarterback Alex Smith went through limited participation in 49ers practice Thursday and appears to be on track to return to the lineup Monday night against the Chicago Bears after returning from a concussion.Meanwhile, Bears quarterback Jay Cutler, who also sustained a concussion on Sunday, did not practice. Veteran Jason Campbell will start for Chicago if Cutler does not receive medical clearance. The 49ers conducted their practice Thursday under the direction of assistant head coach and special-teams coordinator Brad Seely, as coach Jim Harbaugh underwent a "minor procedure" at Stanford Hospital to deal with an irregular heartbeat.Harbaugh is expected back at practice on Friday, the team announced. All signs point to Smith returning to action Monday night after receiving clearance to return to limited practice.Smith sustained a concussion during a successful fourth-and-1 quarterback sneak on Sunday in the second quarter against the St. Louis Rams. He remained in the game six more plays, including a 14-yard touchdown pass to Michael Crabtree.Smith experienced blurred vision after he buried his head into the pile, where he was met by Rams middle linebacker James Laurinaitis. When the 49ers took possession again after the Rams were held to a three-and-out, Smith was removed from the game. Colin Kaepernick entered and played the remainder of the game.Kaepernick completed 11 of 17 passes for 117 yards in the 49ers' 24-24 tie.
