Brian Hoyer's stay in Chicago was short-lived.
Less than a year after joining the Bears, the veteran quarterback has found a new home.
According to NFL Network's Ian Rapoport, the San Francisco 49ers and Hoyer have reached an agreement on a two-year contract.
Earlier Wednesday, CSNChicago.com's John "Moon" Mullin reported that Hoyer was at Halas Hall this week cleaning out his locker and say his goodbyes to his former teammates.

Hoyer, 31, started five games with the Bears last season and tossed six touchdowns with no interceptions and had a passer rating of 98.0.
